Flu Mist Vaccine
For the third year in a row, Wesleyan Health Services is pleased to be able to offer the opportunity for parents to have their child(ren) receive the Flu Mist Vaccine during the school day. Physicians from Pediatric Associates of North Atlanta will be on campus to administer the vaccine on two different dates. We have set aside Tuesday morning, October 7, 2008 as the date for Lower School students. Tuesday, October 14th will be the date for both Middle and High school students and it will take place during their lunch hour.
Please see the two consent forms belowi If you want your child(ren) to receive the Flu Mist vaccine, both forms need to be completed and submitted with payment. These forms may be turned in to any school office or either Infirmary. The cost is $30.00. The third attachment is information from the CDC pertaining to the Flu Mist Vaccine. Wesleyan School has two Infirmaries staffed by Registered Nurses. They can be reached via e-mail at schoolnurses@wesleyanschool.org.

| ANNUAL PHYSICAL EXAMS |

Wesleyan School's Administration and Medical Directors require that every student undergo a physical exam prior to the beginning of each school year. Physicals are to be obtained and dated between April 1, 2008 through August 1, 2008. The completed form must be returned to Wesleyan Health Services by August 1, 2008. Please mail forms to Wesleyan School, Attention Health Services or they may be dropped off in the Administrative offices in Gillfillan Hall during the summer.
All 7th through 12th grade students must complete the Georgia High School Association (GHSA) Mandatory Health Questionnaire and Physical Form. If student is participating in a fall sport/activity (football, cross country, softball, volleyball, cheerleading, or band/color guard), this form needs to be on file with Health Services by Augist 1, 2008. Coaches will not allow athletes on the practice field after August 1st unless this form has been submitted.

| PRESCRIPTION MEDICATION AT SCHOOL |

If your child will be taking a prescription medication at school on a regular or "as needed" basis, you must submit an Authorization to Administer Medication During School Hours form. Physician and parent signatures are required. See form below. | |
| HEALTH CARE PLAN |
|
If your child has a history of any chronic medical condition, please indicate this information on the health form. These conditions could include asthma, diabetes, seizures, severe allergy requiring EpiPen etc... We have care plans available for several of these health conditions and would ask that you complete one, if it pertains to your child. If you have not already recieved one of these forms, and need one, please call or e-mail one of the infirmaries. We have included a few of these on the links below. | |
| IMMUNIZATIONS |

All new students and rising 6th grade students must submit a current Immunization record. Immunizations must be recorded on Georgia Form 3231, according to State law. This form can be obtained from your physician or the Public Health Department. No other form is acceptable. A returning student's form should already be on file in the infirmary. However, if any vaccination or booster has been given to your child, an updated form should be submitted. | |

Information on MRSA
As you know, there have been increasing incidents of reported MRSA infections among athletes athroughout the country and the Atlanta community. We have not been made aware of any recent cases here at Wesleyan School. Methicillin resistant Staphylococcus aureus (MRSA) is a type of "staph" bacteria that is resistant to many antibiotics and frequently causes skin infections. It often looks like a spider bite, boil, abscess or turf burn. MRSA is generally spread by skin to skin contact or by direct contact with the infected wound drainage. It may also be spread by contact with contaminated surfaces of things such as sports equipment or personal hygiene items. We cannot stress the importance enough of good hand hygiene. Frequent hand washing is the best way to prevent MRSA.
We recommend that if you see an early sign of skin infection in a cut, such as redness, swelling, pain or pus, that you contact your healthcre provider and have it evaluated. Early detection and treatment is important. Below is a list of ways to prevent transmission and reduce the incidence of MRSA.

Wash hands frequently with soap and water or use 60% alcohol-based hand sanitizer

Encourage good hygiene by having your child shower directly after practice and games/meets

Change sheets and towels regularly

Do not share personal hygiene items scuh as towels and razors

Avoid contact with skin infection of others

Have your child report any suspicious skin lesions/scrapes etc. to parents

Make sure any exposed wounds are covered with a clean, dry bandage taped on all four sides
-
For girls, avoid manicures/pedicures if you have any small cuts or open areas on your hands/feet
